If you are going down south, it can be a tad bit of a problem, but for the most part you'll be fine. Many in Madrid/Barcelona speak English. As someone has suggested, download the google translate app, it'll come in handy. If you've an unlocked phone, you can get a local SIM card instead of paying your carrier for international roaming (at&t charges $10/day if you've the international day pass). Consider paying your carrier for the first day if you can't get good cheap SIM card from the airport and shop around locally.
I wished I'd picked up a little bit of Spanish before the trip, but it's on my todo list. Learning French at the moment. Consider learning Spanish, it'll come handy everywhere!

That said, they won't get much cheaper than this... so, if you're planning to go, no reason not to jump on it. Did two weeks last year (Madrid - Toledo - Granada - Alhama de Granada and Costa Del Sol - Seville - Madrid) and had a blast.
In the large cities like Madrid you'll be fine but I'd suggest picking up a basic data plan for your phone so you can use google translate if needed. I know basic Spanish and got by on my AT&T travel data package (I think it was $20 per billing cycle in 2017) for my two week trip to Italy and Spain.
