Yes, does not work, went to the store and was rejected. Their system does not allow to go to next steps with the plan. Was told I could change my plan but didn't want to.
Thank you. Saved me a trip. And simple choice is priceless I wouldn't change it for any iPhone too.

Is it $580 flat for all eligible phones or varies based on whether trade-in phone is 6s vs. 8 plus?

yes, but you'll usually need to reactivate the canceled line if you try to get another deal like this within 90 days of cancellation.

all plans should qualify. for simple choice the costco rep has to call t-mobile with a "token" code to add the line as their system doesn't allow it.
Do you have to buy the phone out right to get the gift card or can I finance and wait for the gift card and use that to finish paying it off.

Hi. Did you add a line to the existing account or just opened a new account? Costco guy at my location told me that you cannot cancel a line 90 days before and 6 months after the purchase. And also told me that the new phone I take will be locked to the new mobile number we take. ☹️
The only truth - the 90 days no cancelation. The rest - not true. You can switch the number on the sim card. Or you can use different sim card (e.i. different number). The only requirement is that you have to activate the sim card that the phone came with. Otherwise no rebate. Activate the phone line on the new iPhone and then put that Sim card aside if you don't need it. I did this 3 times last year for all our lines.

Ideally you want the lowest trade-in value that shows up as the one time bill credit if you are looking to cancel as early as possible (which I believe is 60 days). Vs trading in a much higher assessed phone which would then take more months for the bill credit to be used up.
If you close your account and you have balance - they will send you the money on a pre-paid card but it takes time - about 2 months. Bottom line - you don't lose your money.

iphone 6 does not work for this deal (it is 6s upward). It probably will not work for the BF deals either.

Flat. If you have a 8 plus you are better off selling in separately and buying a 6s for the tradein

Should be 90 days. Says so in membership wireless page. https://membershipwireless.com/offer8

Went to Costco earlier today and the manager claimed that I couldn't take advantage of the offer since I had 4 cancelled lines. I've only cancelled one line on the account since inception, and that was well over 90 days ago. I took a glimpse on their screen, which linked phone numbers and devices I've never owned or used to my account. They also claimed that they couldn't tell when the lines were cancelled, and I would have to reinstate all 4 lines, some of which weren't even listed as voice lines (one of the lines had a Coolpad surf listed). I didn't put up a fuss, and will be going to a different store tomorrow to try my luck again. Apparently people have been banned, so I'm glad I didn't push it. Really irritating though.
I have seen in the store that the numbers I received before I transferred my phone numbers show as cancelled lines. Did you transfer you numbers when you opened your account? If this is it, they should not take those into consideration. However it is probably impossible to persuade that manager since, it appears, he has made up his mind. Try again explaining this (he can verify it I suppose) and if not - go to another store or try another employee on a different shift.

If you are not getting this deal I personally would not recommend you get a 6S either. The next deal may be 7 and up. In fact I am surprised this deal includes the 6s. The last one I did only allowed iphone 7 upwards.

Don't think I was clear. I currently pay for two lines. My understanding is I'd have to add a third to get this deal. Could I immediately cancel one of my current lines after adding the third to my plan as long as I'm ok with getting a new number etc.
As they told you in the previous reply you cannot cancel any lines while waiting for the card. The determination whether you receive the card or not is made by T-Mobile employees long after you have sent your trade-in phone in. If, at that time, you have cancelled a line (even after you have made the purchase) you will not receive the promo.

Is there any reason the phone actually has to be active on the lines while waiting for the rebate? IE, if I switch to Tmobile and have family members that would like to keep using their existing (compatible) android devices, can I just swap the sim card? Usually everything is associated with the line rather than the phone, but I wouldn't want to jeopardize a rebate this size.
You have to activate the sim card at least otherwise you will not qualify. What I am trying to say is that you have to at least make a phone call or send a text message from that sim card (regardless of what device you are using). I believe they look at the date you first used that line to determine if you qualify to unlock your phone. There is minimum of 60 days (not sure, could be 90 days long) before you can request phone unlock regardless of whether the phone is paid off or not.

See above post - paid off phone and a timeframe of using the service. They look at the phone line that the phone came with. You can use the phone with any line, though. And they look at the when you used that line first. That's what they did with my 3 phones last year. I used the extra numbers for T-Mobile Tuesdays rewards.
