Thanks for all the posts with useful information. Just came back from my local Costco and want to share the experience:

Background: iPhone 6s at H2O wireless, not T-mobile customer previously, only have one line to port in (expensive...)

What bought:
iPhone 11, 64GB, White: $629.99

Plan signed up:
Magenta $70/month
//reason for not choosing Essentials is about it is only $10 cheaper and don't have Netflex and Essentials $60 doesn't include tax and fees while Magenta $70 DOES.

Activation fee:
Per Costco booklet, there is no activation fee, but it is Tmobile policy or Costco will reimbursement. Anyway, I guess you don't need to worry about it.

Monthly charge:
I signed up Autopay. I remember $70/month price requires that.

Port in:
For my case (H2O), I called H2O customer service: 800-643-4926 and verified my recent 3 call/answer numbers. They then messaged me my H2O account number and told me the PIN over the phone. It turns out that's all you need for port in. BTW, even though I left the store for a few hours, my old phone still work at H2O so the port-in may need a whole night to complete.

Trade in:
They print a shipping label (no box or envelope supply) and you are supposed to ship your old phone within 30 days. You can NOT do the trade in at local T-Mobile stores.
For my case, they entered the IMEI number and estimate it cost miserably $36. The associate told me that that $36 will come in as statement credit while the rest ($580-$36) will be sent via gift card rebate.

BTW, judging from the fact that it is a flat $580 regardless of what phone you trade in, isn't it stupid to trade any phone other than the lowest tier, namely 6s? Any phone better than that, you won't get extra value. Maybe this deal is exactly to allure the shabby SDers...

Get the promotion
I was told by the associate that I need to wait 90 days before I can cancel the line and switch carrier (I assume by then I will also get both credit and GC). Anything earlier than that will either forfeit my promotion or, if I get the GC/credit, they will bill it back. Also, that's the timeline that Costco will be paid by carrier for the transaction. For safe, I may be willing to hold 3 months before I change anything. If so, here is all the money goes:
1. Monthly charge: ($70-$30)*3=$120 , $70 is T-mobile price, $30 is H2O price
2. Phone cost: $630-$580=$50 (I bought at NH, so no sales tax)
3. Trade in: iPhone 6s.
So essentially, you spend $170 and trade in a iPhone 6s. After 3 months, you get an iPhone 11.

If you add one more line ($50/month for Magenta), the cost will look like:
($70+$50-2*$30)*3+(630-580)*2=$280
Or say, if you trade another iPhone 6s, the 2nd iPhone 11 will cost you net $110 (=$280-$170). Surely the more you trade, the better average you get. I think the line limit is 4/account.

Good luck. Let me know if there is anything I calculate or think wrong.
So, the Costco agents say that we cannot cancel the connection/new line unless you complete 6 months. Is that true.?

Yea... what can you do with $245 these days anyways.
I can do more with $455 if I didn't have to pay 3 months of Magenta Smilie Or just not upgrade and save the cash entirely. Hopefully they will have another ~$650-$700 promo in the coming weeks.

To be specific, they did take my Costco card because they needed the membership number during the purchase process of the phones.

That being said, I don't know if the name of the Costco card holder and the T-Mobile account holder has to be the same. I doubt it.
Nobody cares who paid for the phone as long as the receipt date is valid (within promo period)!

Does it makes sense to buy a used 6s for $180 and trade in??
Nope ! Too expensive.
Rather go to totalwireless.com, buy it for $80 + $50 prepaid card, use code NOV25 for $20 off and pay another $15 for 1 day shipping (if you feel so) otherwise 2 day shipping is free anyway!
Then you can sell the $50 prepaid card for $35 or so on CL or eBay !

I have attached the offer terms in the Wiki and, if you have done the purchase, you should have all the documents. Please point out where it says the rebate will be clawed back if you cancel within 6 months? Please don't be ill-advsied, people. When you see so many reps can make mistakes, why you trust this specific sentence rather than the documents in your hand (and when many others are NOT told so, such as me)? Even if you go to small claim court, I don't think they can't take verbal words as evidence against you.

My estimation of holding is 2~3 months. The reason?
1. Two full months to qualify the unlock
2. 8 weeks for proceed the promotion (assume you immediately ship the trade-in after purchase. Late shipping can add more turnaround)
Alternatively, you should leave when and only when you unlock your phone and get all the rebate.

Both numbers are from the documents I posted in Wiki and I have read all the sentences of those documents. If someone is going to suggest otherwise, please point out your concrete evidence but please not "I'm told so"

AFAIK unlock happens after 40 days... I have unlocked at least 6 phones in that timeframe. Also my virtual card showed up in less than 30 days for the last trade in.

Does the accountholder for the account where the new lines will be added, need to be a costco member or can one of line users just be a costco member?

Also can tmobile one amplified get in on the deal?

That's great and highly welcome. Essentially without contract limit, people can leave as soon as both targets are hit. What I say doesn't exclude the possibility of early finish. But people shouldn't expect something even later than those writteen on paper

Just did the deal tonight for a 64GB and traded in a 6S. Associate put me on the Magenta Unlimited 55 plan even though I'm still over a decade away from qualifying for that :-0

Going to transfer t-mobile SIM from wife's 7 to the phone and disconnect the line once I get the rebate. I got the shpeel about the 6 months but was not asked to sign any contracts so that's not going to hold up.

I see where it says the One plan qualifies. I'm on One Military. Is that included?

Though it's not written anywhere, but Costco doesn't sell above 64GB.

With Costco, you get very good deals, but at cost of selection. They have very limited colour options and memory.
The last iphone promo I purchased an iphone Xr 256gb from costco. Very rare but they do sell larger sizes.

Question: what if Costco account is under one name and T-Mobile account is under a different name? Will this void the promo? Anyone has experience from previous $750 back promos?

I did a promo back in May (opened a second T-Mobile account for that) and now they would not allow me to open a new account under the same SSN.

Stuck on a TMO One promo plan (100 for two lines plus one line free). If I touch this plan, I'd lose a free line. So frustrating.
Dude, if the promo is different, you can jump on it !
Since this promo just started 2 days ago, obviously your prev promo from May cannot be the same one so you are good to do it one more trade-in now bu adding a new line!

Just tried to add a new line but system says I am on flexpay. So, the rep was unable to add a new line. Anyone know what that means?
