For those in the Seattle area, I did an online chat with Costco reps on Sunday and the stores that had TMO iPhone 11 stock were Tukwila, Redmond and Kirkland. None of the Seattle stores had stock. I asked specifically about the 64GB 11 (possible Seattle stores had other versions, but considering how rare they seem to be from this thread, I doubt it).

Also, to clarify a few other things:
1. The rep required that I have the IMEIs of the trade-in phones - he entered them into his system right there at the kiosk
2. The rep also added the $15 monthly insurance to both the phones, even though I didn't ask for it and he never mentioned it until the deal was done. Annoying, but I was able to go online, create my TMO account and then login and cancel the insurance.

The Costco rep also F*** my account he created 2 individual $55 account instead of a Family share plan.​ I had to call T-mobile to combine them

Answer is simple: there's no credit, device HAS to be paid off (owned by you) in order for them to accept it as trade in.

The new device— not the device being traded in. I wonder if the deal is adjusted to be full price purchases only.

No you'll have to add new line and cancel it after you get rebate (2-3 months of service), if you cancel a line before trying to get this deal you will have to reactivate it anyways (any line less than 90 days old).

Yes you pay full retail (minis any discount given by Costco which is usually $50) plus tax if one is due on each phone before you leave the store.
