2. How to Redeem Your Offer

The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

has anyone heard this, that credit check fails or goes bad once you freeze / unfreeze your credit? I last applied for a credit card 24 months back!!
I paid in full, walked out with a phone, the guy said I will get the full $580. And the email I received from T-Mobile said my 6S trade in was valued at $31 lol.
But the Wiki Board on first page says "Purchase the device through our monthly payment plan"

You need the IMEI. It is printed on the RMA label that you include with the phone when sending back. So no, you can't use your friend's 6s.

Went for the 64gb 11. I specified exactly what I wanted to do and the guys seemed knowledgeable.
New Phone 11 rang up at 629.99.
$33 for trade-in.
Did the Magenta Plan.
So should be $547 on the prepaid card coming back.
Everything went smooth, ported number over, got shipping label. Will send my old phone back in a few days. Overall process took a little over a hour. Hopefully everything goes smooth with rebate card then I'll jump ship and go back to Cricket in a few months when everything pans out. $70 for a phone plan is ridiculous compared to the $35 I was paying on Cricket.
