2. How to Redeem Your Offer

The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

You don't have to use your apple ID. When that screen comes up, click the new account, etc. In there you'll see an option to not create an account or create later...along that line. Then just manually select continue and your phone will be at the home screen with no apple id link to it.
