Most of the questions asked here are either in the promotion posting, or are just stupid. Read before posting.
The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

If so, these are known to bounce around the U.S. due to that warning.
If not, it probably just got routed wrong.
they didn't give me any shipping box with battery warning sticker. should I be concerned about that or just use my own box to pack it and ship?
1. If I add an additional line to my existing plan, can I have the cost of the new phone distributed into monthly payments, or do I have to pay for the entire phone upfront?
2. How exactly is the $580 rebate paid? Is the "trade-in credit and Virtual Express Prepaid Mastercard Card" a prepaid gift card that can be used anywhere? Or is some portion of it a credit on my phone bill?
Personally, I have left the line open for minimum 90 days and then cancelled. It was worth it because for my simple choice, the extra line was $10. On a friends account that I did, it was $20 a month per line.
It's worse when users go in and flat out ask "so when can I cancel"
That raises a big flag.
Just go in, do the deal, pay and leave. No need for all these questions.
I also had a mismatch of what rep and chat rep said about inventory. They had 5 in stock about which the kiosk rep was not updated. So, I request him to check with management, and then after checking, he told me his list had it wrong.

Chaoss is referring some professional shipping company/person will put a battery inside warning sticker when they handle battery-contained shippment. It is a practice there, but, for this promotion, you do NOT have a sticker and do NOT need one as well.
What you have sound right: you only have a prepaid label. It is your responsibility to find proper packaging material, say a box or bubbled envelop to ship your phone. Costco will give you no shipping supply to ship your trade-in phone.
