Most of the questions asked here are either in the promotion posting, or are just stupid. Read before posting.
The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

Total charged was $899+ tax for 64GB iPhone 11 Pro. Shouldn't it be $949+ tax?
I had a Costco rep that assumed he knew everything about this promo, was incessant that the label was available on the T-mobile website, and refused to go back after the fact and try to initiate the trade in label because he was too busy. Even calling Wireless Advocates confirmed the label needed to be printed in store and to tell the rep that he could call their support to walk him through how to do it, but again he completely refused -- I don't know if he thought of it as an insult or what.
Either way, I saw where someone said they were able to receive a label through Wireless Advocates email support after the fact, so I gave that a try and that rep's response was that "this promo's trade in is being handled by T-Mobile so fill the rebate online and it will print out a label for you" -- reading everything here and having done this during last year's promo, I'm 99% sure this is not the case. I responded to that email and asked for it to be escalated since this is conflicting information between their own support reps.
I'm worried that anything done through T-mobile will result in the bill credits instead of the prepaid card, so if there is direct evidence to the contrary, I'd much rather go that route.

$360: Sign up 2 lines on Magenta and pay for 3 months of service then cancel
$1260: Buy 2 iPhone 11 64gb phones at the local Coscto (if they have them)

Basically comes to a net of $375 + $30 tax per phone.
Worth the hassle? I suppose a bit better net by about $70 per phone than the launch day upgrade where we didn't have to add lines. And obviously better than straight up retail from the Apple Store. I just wonder if there will be something even better closer to Thanksgiving when this extended promo ends.
However 1st phone, I never got an email reminder with trade offer. However I did get the print out to ship both phones at store. Anyone else have the same issue or should I be worried?
