2. How to Redeem Your Offer

The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

However, given the amount of contingencies in this deal, I foresee having to argue for the rebate with T-Mobile when the time comes. I encountered all kinds of roadblocks a few years back when I submitted BOGO rebates. Unfortunately I didn't seem to learn my lesson.
Not yet traded in and not yet received any promotional credit. So not a guarantee.
My iPhone Pro Max rings are $999 ($100 off) at the counter. Added a new line on Simple Choice (manager says it qualifies otherwise it can't proceed). Added a local? Promo of new line $125 Costco GC.
Now waiting to trade in an iPhone 6s within 30 days and see the updates on rebate.
Might be risky but the manager is very willing to help if anything happens.

Costco: Hi, at this time we're showing this phone as sold in select stores and cannot gurantee availability. Howevet, I can refer you to your local Costco to check additional options.
When I talked to the Assurant folks, they said "not a big deal" since the IMEI is correct, they will assess the phone anyway and send me the new offer to accept.
But to save the processing delay later on, I still went to Costco and got it corrected, they gave me a new paper with new shipping label and RMA receipts !!!

One of the condition says:
You have requested no more than two mobile device unlock codes per line of service in the last 12 months
Does it mean that if I buy 4 phones on a line, I won't be able to get all 4 unlocked because if I get the 4th phone unlocked, I won't meet the condition of 'no more than 2' devices in the last 12 months.
