You didn't get those cause he didn't process a trade-in cause you didn't have one. You will have to go back when you have a trade in so that he can process it and print out the subsequent forms.
Do I need to go thru 'return' and 'buy' process again? or they can simply process trade-in now? I bought it from Costco different city as it was OOS at my Costco. I hope I can process it in our Costco and won't have to travel far again?

I got the H20 sim from BestBuy. When I enter all the required fields for porting and sim number. Then I see the Mobile plan pin. The minimum is $10 these days.

Yeah, the H20 cards are only $1 at best buy. You buy the $10 plan and it costs you a total of $11 to port out a number. Not really a big deal, and has worked with no problem in the past

Costco only deal for the mastercard $580 rebate delivered within 8 weeks.

if you go through T-Mo directly, still $580 rebate, but in 24 monthly bill credits

FreeUp (AT&T MNVO, real cellular, not VOIP) still has $8 sim cards on Amazon.

you port in and get their free service of 250 minutes/text per month. No cost per month other than you need to login and click the continue plan button each month within 7 days of renewal.

For the folks still hoping to avail the offer and looking for 6s , Boost mobile has a in-store deal of 6s for $24.99 plus taxes with at least $50 plan and port in. This might be cheaper than some of the other options. However, disclaimer: I have not checked this myself and just sharing what I came across in case it helps someone 🙂.

yes, but you'll usually need to reactivate the canceled line if you try to get another deal like this within 90 days of cancellation.

all plans should qualify. for simple choice the costco rep has to call t-mobile with a "token" code to add the line as their system doesn't allow it.
Has anyone got this done successfully with the SimpleChoice plan? the costco rep told me it's not qualify

As far as I know there is no contract and so you can cancel the service anytime you want. Unlocking period might be an issue but if you are getting it to trade in then it won't matter anyway.
