Chase Slate is only 15 months but 0% balance transfer fee.
The slate itself sucks as a card to actually use for anything other than the BT, but if you're mainly concerned about the BT, and the 3 extra months won't kill you, it saves you 3% of however much you're transferring over this... plus once you're done with the BT you can product change it into a genuinely useful card like a Freedom or something.

I used to have this at Bank of America "Shop Safe" and found it extremely beneficial. But they ended it for incomprehensible reason.
Anyway, I've tried to investigate whether it's available at below links, but if the information is there, it's well hidden, I can't find it.

Currently you can generate a number using the web app (no native app for windows/Mac or mobile currently).
Yes, they give you a new credit card number for each VAN which also includes a new expiration date (up to 12 months from the current date) and a new 3-digit CCV.
You can specify a maximum amount that can be charged to the VAN and once someone charges to that VAN, they are the only one that can charge to that VAN again, so it is locked to that one vendor.
I use VANs all the time and have had no problem using them on-line or on the phone. The only time you can't use them is for in-person purchase where you need to present your physical card.
I have several apps for fast food places on my phone (McDonalds, White Castle, etc) and I use a separate VAN for each. Each card is limited to just that vendor and has a low limit. Once I start to reach that limit, I can simply "edit" the VAN to up the limit.
So I am protected from others getting my VANs and using them to charge to other vendors, and if they try to charge to the same vendor, they will be limited to the value I specified on the card.
Of course, if you have charges on your account that you did not make (even with VANs), you can always call Citi and have it removed, but that can be a hassle. Without VANs you'd also have to get a new card since your primary number was stolen. With VANs you can just close out the VAN with the app and you're done.
