Glad I did. $300 is better than $100 and I can still port out in a few months. No contracts on the Kickstarter plan. They automatically waived the activation fee by doing it online as well, but after looking at my confirmation email it says there will be a one-time credit of $30 within 3 invoices however that plays out. My first bill will be charged 30 days from today.
I was concerned with the Amazon reviews saying they were sent 4 of the same SIMs, but it worked flawlessly. Over chat the rep told me which one to use. I also asked if I could double down on both the $300 and $100 Amazon SIM promotion, but was a no go. Worth a shot anyway.

4) makes no sense because if the line was added to an existing account then it doesn't have its own Account Activation Date.

I think the account activation date is not the really problem that's why I don't suggest doing so in the first time. Some ppl even added line to free 1 yr plan account or SWAC account, so account activation date is different from order date, and they are not getting invalid as now. (Which seems kind of weird to me because they are customers awhile ago)

After 5-7 days, I'll check it again. If it still gets Invalid Activation Date, I'll resubmitted a 3rd time using the activation date of the account (not line 3). If that still doesn't cure the error, then it means an add-on doesn't qualify, despite what the sales/chat rep said, and I'll cancel that line.

If you have multiple rebate submissions, use the option to look up by name and zip code. It'll show all of them, then you can click on each one to see its details.

If I resubmit the claim for line 3 again, I'll use 3/14 for its Purchase Date. I said Activation Date in my earlier post because in my case it's the same as the Order Date (for acct 1, line 1).
Like I said, if it still fails with the 3/14 date, my suspicion is that an add-on line (on a later date) to an existing account doesn't qualify, despite what the reps said.

2 lines were ported in from Lycamobile. I fully expect to get the rebate for these 2 lines. These lines have status:

So for those who get status other than "Missing Channel Code", it may be a sign that the rebate may not be approved.
