Glad I did. $300 is better than $100 and I can still port out in a few months. No contracts on the Kickstarter plan. They automatically waived the activation fee by doing it online as well, but after looking at my confirmation email it says there will be a one-time credit of $30 within 3 invoices however that plays out. My first bill will be charged 30 days from today.
I was concerned with the Amazon reviews saying they were sent 4 of the same SIMs, but it worked flawlessly. Over chat the rep told me which one to use. I also asked if I could double down on both the $300 and $100 Amazon SIM promotion, but was a no go. Worth a shot anyway.

Once i setup online account i added auto pay and removed sprint complete. But the first bill unfortunately does not reflect autopay discount even though it says it will autopay after 15 days.
2 lines were ported in from Lycamobile. I fully expect to get the rebate for these 2 lines. These lines have status:

2 lines were ported out + ported in from Sprint free plan within 2 weeks (do not expect rebates) & 1 line ported in from a VOIP provider (not sure about rebate). These lines have status:

So for those who get status other than "Missing Channel Code", it may be a sign that the rebate may not be approved.

I see that you have no problem with line #1, line #2, but problem with line #3, I recommend to use the date you used for line#1 for line #3 since they are under the same account and line #1 is so far so good
If you tried everything and still getting Invalid, then it isn't qualified being an add-on line to existing account with active line
I did 4 lines first. Submitted rebate.
Then i did 1 more line after a week. Submitted seperately.
For both i used the different order dates. The only thing is the last page asks for primary number. For that i used the same main number for both submissions.
All show up fine.

2. Not sure how to explain. First enter the rebate code, then the webpage asks to submit account number and cellphone numbers you have ported into sprint in the order. Then once you hit next it asks for your contact details. Here i put in my name, contact number(the number for 1st line), etc. The contact page i submitted the same info in both cases.
