Speaking to the promotion reps from (800) 999-6389 and they only mention being on the $40 line is sufficient.

Don't know about the rest of you but the $100 rebate does not come back to you automatically. You need to apply at https://promo.metrobyt-mobile.com/. I took me some looking around to find this URL. Metro didn't tell me and the retailer I bought my phone at said to watch out for a text message from Metro that never came.

1. Is the phone $99 + activation + plan or is it $200 + activation + plan out the door?
2. I'm going to remote alaska where this won't work. I need a phone now for a month and I will need one on the way back. However worried that I won't get my 6 months if I need to travel after a month. Any remidies?

It's almost certainly like every Metro deal - it depends on the store you go to. There will be stores that won't sell you the phone unless you get the $50 or $60 plan.
There's a corporate store near me that won't let you get one of those "free with port-in and unlimited plan" phones unless you get the $60 plan as well as the $10 premium security bundle. Any "free" phone ends up being $60+$10+activation+tax.

We have 4 lines— 2 adults use it and 2 teens—we share 3 gigs of data, unlimited talk and text. If we don't go over that, then we only pay $30 for the four of us plus tax. I think it's around $36 with tax. There's no line access fee or any other fee. If we go over 3 gigs, it's $12 per gig. Best deal around, imo.
And I Phone SE 2020 device not available with almost all stores
