Update: finally have the gift cards on hand. Activated two x $250 card. I will get Costco cash card with the gift card. Next: figuring out how and where to move out of AT&T.

So you're got 2 accounts for 1 credit check? I had to check out twice for 2 lines each, so 2 pulls

Yes. That is correct.
For starting the account I added my ssn to match my name.
For port on each line it asks for each of the phone number: pin, account#, name and ssn. Here I put my wife's info

Thanks. I placed an order a few hours ago for two lines, went through fine, but have to wait and see if it have any issue 24 -48 hours later. Hopefully everything goes smoothly, then I will port two more lines under my name, if it goes well, may try my adult kid for two more lines, not sure if this will work, due to lacking credit history. Many hoops to jump.

did your browser show "not secure" at any point during the order? As soon as I click "Add Billing info" it switches to unsecure and never switches back.

I didn't notice any unsecure message on mine, but ATT online registration is buggy, I have to repeatedly change port in phone numbers etc. to finally make it correct, I don't know if this caused cashback portal didn't track two lines.

My acct #1 (wife's name) got one hard pull on its first order, which was cancelled. The 2nd order had no hard pull, and was approved.
My acct #2 (my name) got no hard pull, and the order was approved.
I placed an order for acct #3 (daughter's name) on Sunday and there was no hard pull. Don't know yet if it will be approved or cancelled.

There's no "clawback" clause like Verizon does, but if you want to be sure, cash out the cards (buy money orders with them) or spend them.
