2. Go home and call Tmobile to order Pixel with trade in on new added line.
3. Fill out Costco rebate.
4. Should see credit to make line free and credit to make Pixel free.
How about port in number? Do I port in a number during step 1 or should I call Tmobile after step 1 to port in my number? Will this invaild Costco $200 rebate?

I tried calling TMO and the agent indicated that I would have to add the line AND order the 4A with him at that moment, in one transaction. Otherwise, I would not be eligible for the free phone.
Would you mind indicating how you went about ordering the phone? As an upgrade online? Ordering on the phone? Thank you!

Here's what I did today.
I added a line at Costco today, within 1 hour I got on a call with TMO and asked about the Pixel 4a promo. I mentioned that I just added a line today and that now I need to add the pixel promo as well. The rep told me no problem, since both promos are being added within the allowed time frame. He confirmed that I will get the full $500 credit over 24 months for the pixel 4a with my pixel 1 trade in, and the line I added today will be free. Everything worked out perfectly.

You have to use the IMEI of the phone that you brought into Costco for the promo.

Your safest bet is to call. Try telling the rep that you added a new line but when you try to add the pixel promo, it only gives you half off instead of the full value.
