That is suppose to be a universal sim (but that's what they said about the last universal sim too)
You can also try to buy the Boost Mobile 4 in 1 Sim activation kit (includes the SIM 516TQ). I got a few from Walmart for $5. It's behind the lock desk under register at my Walmart so you have to ask for it. Or you can ask them to look up Walmart # 576363277
