Mint mobile agent just told me that tomorrow will be a Black Friday sale for new members of buy 3 months get 3 free... But the device you're using cannot have been active on Mint Mobile for 91 days. So even if you order a new SIM or phone number it's the device you are using that will determine if you are eligible for the sale price.
So if you are using google voice and renewing with a new number the discount might not work.

Were you an existing mint customer prior to 90 days when you activated the sim?

In any case, i reversed the CC charges and ordered again and they came just fine.
Just don't leave yourself vulnerable to their CS if you can help it. now that i can just get sims at best buy i don't have to deal with mint directly at all.

i'm not sure if they automatically use the credit (they list is as $45 ) to extend 3 months or if you have to set up autorenew for it to be used. i'm waiting and watching to see what happens.
