Get an unlocked phone (I got an old one phone that is not actively used/shown on the T-Mobile accounts page) and add a line at Costco. The Costco rep will give you a print out of a rebate instructions with the rebate #, IMEI of the phone, and the new phone number. I attached a photo of the print out.
Go home and port in a number. Submit rebate online for T-Mobile's $100 and Costco $200 using the port-in number within the 30 day deadline.
Thanks for the picture and clarification. I wasn't sure what number I had to use to submit the Costco rebate if I decided to stack with the $100 port rebate.

From past experience it is 14 to 16 weeks for rebate to come from costco

How important is the IMEI? I plan on using the IMEI of a phone that is getting traded in soon. I'm guessing its just a placeholder number they need for the form and they don't really check?

You just need imei number in costco when adding a line, and i will suggest keep the sim in the same phonefor few days and do some activity like calling
i checked with Tforce and Google Voice port-in is eligible!! Quote from Tforce on twitter:

At the Costco kiosk, would I be able to add myself into my family's plan? I have the account number, pin, primary account holder's SSN, etc.
I remember calling T-Mobile about if it was possible to add someone who isn't on a T-Mobile plan yet as an authorizer user and they said it was possible, would I need to do that?
I don't completely understand your question. I am not sure if an unlocked phone that is currently being used on an existing T-Mobile line is ok for the BYOD promo. I had to physically bring my spare unlocked phone to show the rep the IMEI number.

For now, i moved that sim card (currently on imei phone 1) to different phone. I doubt this clear imei of tmobile network/record. But, nothing to loose . I was initally to get the free line add directly from tmobile rep (tforce)

What details you give to port out Google voice number? The number and what else? I remember back then that there is some default account number something. Thanks
Correct except you should tell Costco that you're adding a free line because for my account, Costco's system couldn't add a free line due to an incorrect "Flexpay" (meaning pre-paid account) error; I have a post-paid account. The rep didn't know what to do and after searching Reddit, I called T-Mobile's retail support line at 888-310-8369 for the rep to tell a T-Mobile rep to add a free line. Don't the T-Mobile rep that you're going to port in and apply for the T-Mobile $100 rebate later though!
Buy a Verizon/AT&T/Boost Mobile number and port in the number after. Submit both rebates online.
Not sure about porting in at Costco. My rebate is still pending and I ported in after getting home instead.
how do you 'buy' a boost mobile # without buying a phone and all that?
Also is the free line unlimited talk text data (my current plan is that) or will it just be unlimited talk and text? Do I have to tell Costco that I only need unlimited talk & text and no data?
