I've got the same question. I visited Wireless Advocates and added a new line/ported in a number from Google Voice. I've now got the rebate paperwork in front of me which states, in part: "Device must be active for at least 3 billing cycles." Can anyone speculate if the IMEI needs to be active, or do they just care about the line?
I just came back from Costco and am reading the $200 rebate requirements.
It mentions "Device must be active for at least 3 billing cycles".
Does this mean this phone must be on the T-Mobile network for at least 3 billing cycles with the sim for the new line in it? Or can the sim just sit in my drawer?
The BYOD device does not have VoLTE so come January, it'll be useless on T-Mobile.
Shoot you got kickback added on? I thought they couldn't do it anymore. I have it on all my other lines. Was able to add the one plus promo without issue.

Hopefully someone can provide insight. I e-mailed them to see if I can switch IMEI (reply isn't until 2 business days), since the line is for my sister - who is going to use her own phone.

Could you provide more detail on how to ask the rep to do it? I asked before, the rep said the kickback no longer available, so they could not add it to other (free) lines. Thanks.

Can someone who've done similar rebate at Costco in the past confirm if it's ok for the IMEI# on Costco's rebate info page (that they gave when activated line) does not need match the MEI# when we fill out the actual rebate?? I would imagine quite a few people changed their mind and use a different phone then the one they told Costco.
I'm new to read this promo. Can someone quickly let me know what should I do? I joined T-mobile on 10/18 and ported in my existing two AT&T numbers to T-mobile. The rep gave me a third line(a new number) for free at that time. So I have three lines in my T-mobile account now. How should I take advantage of this Costco promo?
I ported-in the number after coming home. Do I use that number of number printed on the rebate form for Costco rebate?

I went to Costco and did the add the line and ported a number from Mint Mobile. I got the forms that detail how to get the $200 rebate (T01491). I have read that the $100 rebate for port-in (2020 Switch P2) is automatically done. I wanted to confirm that this information is correct. Also, how can I verify that the $100 port-in rebate is applied? Will there be something I can see through my account status, or some other way?
