The paper and form are clear. 5i and the web form do not ask for the activated number. Anytime either the form asks or the sheet tells you to fill something out with provided info, they point out exactly what and where to get it from. 5i and the web form only tell you to enter a permanent wireless number, not an activated number and not the associated promo number.
The paper form and website are not clear actually. No where does it say enter your "contact info" permanent number as you implied. The Costco rep explicitly advised me to enter the permanent activated number on the form for online submission.
I also explained that the wireless number you submit is used to identify and verify your new line, not to call and contact you. Although, it'll send a confirmation text and number to the wireless number you submitted with.

Costco reps have said all manner of things, just read the thread. It doesn't matter what they say, it matters what the printed paper and web form says. Both are very clear as regards to our newly activated number. They don't ask for it to be submitted anywhere
Why don't you check your rebate status? It will show the details of the rebate submission. That way, you can see if it was submitted correctly or if there's some kind of mess up with the control number (someone else mistyped your control number).
I tried all possible combinations, but no record found. Does it show up immediately?
Got it.. do we need to call t-mobile to make sure it's free line?

I could never find the line promos listed in the T-Mobile app. So just go to this link below in a browser and it should show "2020 Line on Us 3" which is this free line promo. If you have your new phone number working and the sim card activated (like it would be at Costco), that's usually when the free line promo shows up. If it doesn't show up, I would still wait a day or two and check again before I would message T-Mobile to confirm.

T-Mobile's official rebate is similar. It says to submit the permanent number of the new line. There's no good reason to risk a $200 rebate by using any wireless number not associated with the new line.
So, you don't plan to follow-up with them? I won't let that $200 go after all this effort, just not me.
That happened last month. I wasn't aware, but I called and she manually gave me new control number matching with phone numbers, which allowed me to submit the rebates. We'll see what happens after a few more weeks.

Yes. From the info that you've provided, it seems most likely that someone else mistakenly typed your unique control number when submitting their rebate. You should contact the rebate customer support to fix this.
