I am planning to get both 200$ Costco card and 100$ rebate from t-mobile. Anyone know which permanent wireless number I provide during Costco 200$ submission - the one activated at Costco or the ported number?

Yes. The rep from Costco was telling me to ensure that upon submitting the rebate forms for five lines to ensure that the control numbers matched the phone numbers. He says not worry about swapping the sims but to make sure that the control numbers matched the phone numbers as they are noted on the rebate forms.
I did this a couple of days ago and got charged $20 for assisted support. I thought that fee was waived at Costco? Can anyone help me confirm if that was is the case?

Port in at costco or with tmobile is the same. It is the end result that matters.
Costco's does not required port in. It is BYOD. Port in is free line.

It's been way over 24hrs since I added a line at Costco and I still don't see the 2020 Line On Us 3 discount on my plan. I msged tforce beforehand and afterwards to confirm it's a free line for my plan. Anyone else has this issue?
more than 36 hours and mine still doesn't show the free promo... but they all confirmed its free at their end...TMo backend IT system sucks.

I have couple of questions would appreciate if someone can help me with answers.
1. The deal says upto 540$ off so is it I will get 540$ off if I am buying iPhone 12 Pro max and lesser if I am buying iPhone 12 Mini?
2. Does this 540$ off is upfront discount( prepaid card) when buying 2 phones?
