So I filled out the rebate with the new temporary number because that's what was on the phone. I realized later it said to use the permanent activated number and I had since ported in another number. There was no way to change it on the website. I emailed the rebate company explaining the situation and asked them to change it [email protected]
.
Worth a try if you're in the same boat I was.
I'm still not entirely sure which number is "right" to use the whole situation is unnecessarily confusing. I laid out the whole thing and explained it and gave them both numbers so hopefully will be all good.

I did not put in he ported in number or the new number given by costco, Instead I put in the best number to contact me for the rebate as that's what the prompts lead me to believe. This was also used when the promo site texted me about my rebate status immediately after my submission. The only cell phone number you put in seems to be next to email which i think should mean contact info.

Yet, I was told that I had to match the control number with the IMEI number since I had more than one rebate
Same here. I added five new lines. The Wireless guy made sure to point out that I needed to use the correct imei to match each control number when submitting the rebate.

Finally got my eBay number a full day and a half later. Just chatted and got the port done. Decided to spend more than risk a denial for the rebate using GVoice or Boost for the port in.

So free line shows corrects on my account and got kickback and one plus promo active on it.

At this point just need to fill out the Costco and T-mobile forms.

Not sure if I wanna bother with the pixel since I would need to buy a S7 or something off eBay for trade-in to get it at no cost.

Can you share what plan do you have?

I have a simple choice unlimited and I'm sure I qualify for this Costco rebate offer.

When Costco rep pulled up my account, he saw "Corporate Discount" on my account and he said he cannot do it and I have to activate it in the store.

When I contacted T Mobile, they say they don't see any corporate discount added to my account. I had corporate discount before, but I'm not availing it anymore. T Mobile is not sure what to remove as they don't see corporate discount. Costco rep sees it.

Did you have to talk to TMobile rep over the phone or Costco rep was able to deal with it?

Also, do you know if the Free Line and $100 TMobile rebate can stack on it? Again, Costco rep said it is not stackable.

Any inputs will help us all in similar situation.

TIA.
I am on the T-Mobile ONE Plan which was changed over from the simple choice like 2 years ago due to some weird thing that we needed to do to get more discounts.

When I approached the Costco Rep I specifically asked about the add a line offer with the $200 rebate. He said "sure, let me pull up your account and I will add it." While he was doing that I also inquired about the free line, and he said that it would be the line that I was getting. Typed in the SSN and last name and it pulled up the account and it said "Corp Disc" in one of the columns. He clicked next and it said to call in. COSTCO rep called it in, gave a bunch of numbers, passcodes, and a whole lotta whatever he said and wrote down my new number, and passed me a sim card with the number written on a sticky note attached to the sim card holder thingy. Gave me the paperwork to sign about all the rebates, that I have to give them my first born child, my soul will be kept in some dark corner of the universe, etc. etc. walked around to shop and left.

I have not tried to stack the porting in number yet, as I was going to try a Google Voice number, but have heard that it will get rejected, so may get some prepaid on Verizon or something along those lines and go for the rebate.

Some have had success before but only 2-3 months worth of time will tell if it will really work or not.

WORD OF CAUTION: It would seem that if you open the free line and put a phone on EIP on it, it will invalidate the free line, so DO NOT ADD A PHONE PAYMENT PLAN TO YOUR FREE LINE or you will start paying for it. People have been burned by this previously.

EDIT: Added info about signing my life over to the evil mobile phone masters

I have a TMO SC-plan of which I do not qualify for the current free-line offer (let alone the $100 tmo port-in). Having that said, does the costco $200 rebate have any restriction on which plans are eligible? Is there a list of SOC codes that's eligible for this plan as well?
Simple choice or select choice?

I would say then that the rep does not know what they are doing then. I walked out with paperwork in hand that had the control number and the website on where to go to fill in the rebate. Then again the reps at my Costco location have been nothing but helpful and super energetic. This is in Los Angeles, so every penny counts for us.

That is provided you add the free phones which are not eligible for the free line. The only phone I know eligible for the free line is the Google Pixel 4a which I already added on EIP to the free line.. and everything looks good now on my plan.

I am glad to hear it worked out for you. I was thinking about getting in on the iPhone deal, and with that it seems it may or may not invalidate the line. There is SO much information out there that I honestly cannot make good heads or tails out of it. I was just hoping to save someone the trouble of finding out that putting a phone on it invalidates their free line, since promos cannot be combined.... or can they?
