Accd to the costco rebate form, it asks for "activated permanent number". Right now, it's filled in with the temporary number... But I've since ported in a number from tracfone (one of the slowest port-out providers... Takes 24 hrs!)
I will be doing as instructed and giving them the permanent ported-in number. It isn't like the number that the costco rep filled in exists on my acct anymore... So it won't make sense to give it out.
My experience... I got the Samsung S20FE from Costco for $530 plus tax and then I'll get $300 back in Costco cash cards. Once I did that, I then had an available phone. I verified with T-Mobile that I qualified for a free line and that an unlocked Google voice number would qualify for the $100 rebate for porting a number. I then went to Costco. Told them I wanted to add a free line and wanted a $200 cash card for bringing my own device. Once that was done I went home and called T-mobile to verify that Costco added that free line correctly. They confirmed and I submitted the rebate on the T-Mobile website for $100 for porting in a number.
I had actually just requested that Google Voice number for the promo. I got it and unlocked it instantly. The only frustrating part of all these promos was that I couldn't see the settings to set up the Google Voice pin on my computer for some reason (incognito browser or not, Google Chrome nor Firefox). I managed to see it on my Android phone, Google Chrome, "desktop site" but I forget whether it was incognito mode now.
What link you used to submit 100$ tmobile port in rebate ? i cannot find one.
Please help

rebate form does not ask for "activated permanent number" anywhere on the printed sheets nor the web form

totally false info
what Im staring at the form right now. control number on the top right. Costco BYOD rebate form, promo code T01491. This is for the $200 costco shop card.
"activated permanent number" is listed to the left of "kiosk #". Next to IMEI.
Spread FUD elsewhere.
(Edit: I should preface this by saying this is the "T-Mobile Rebate Redemption Form")
Uhh yeah it's listed. No one's asking what's listed. The question is what do you fill out in the rebate form for submission. Nowhere on the printed sheets does it tell you to submit "permanent activated number". No where on the web form does it ask for - permanent activated number. Your post I quoted is completely false
Can confirm offer is working at Richmond, CA costco location. Took 10 mins.
Can you advise if I port in 4 number, am I going to get 4x$200 = $800?
also are you able to get additional $100 port in shop card? I also wonder in order to keep the $200 shop card, for how long am I supposed to stay in t-mobile?

this is correct. both rebate forms you should file with the port in number, not the temporary number. when it will come to process the rebate they will look at whether number is active on your account. and the temporary number will be long gone.
what do you mean "active"? like when they check for the gift card they can look at the IMEI number on that line? the third party can access to tmobile system to check that?
correct. I am not sure how they check but rebate clearly says the 'device' has to be active on TMo network for 3 billing cycles or so..

If this rebate is anything like their usual phone BOGO rebate deals, you will get the $200 at around the 9-10 week mark. Thats how long I had to wait for my Galaxy note rebates.
I would keep the line itself active for at least 90 days because, at least according to some kiosk workers on reddit, if you cancel the line before then, the kiosk will get a chargeback for the rebate card, and then they will no longer let you activate lines through them.
Considering that the line is *free*, theres no reason to cancel it, unless you need to cancel 2+ lines for whatever reason.

This promotion returned today and will end on Dec 3. You can also get a free line at Costco when you have 2 or more postpaid lines, I think this might help since I saw lots of people worried about whether this promotion can be done at Costco or not.
These two promotions can be stacked I believe, based on the community discussion.
