1. You go to costco and tell them nothing else than "I want new T-Mobile account with 4 lines". They will open it and give you temporary numbers and 4 forms for the $200 promo ($200 x 4 = $800 costco "cash" ). You may need 4 IMEIs of 4 unlocked or T-Mobile smartphones. Do not apply online until you have the permanent numbers.
2. Third line will be automatically free you don't have to do anything but have in consideration the discount is not 25% of final bill of $160, but a discount of $20 (12.5% off), since the lines are billed as follows, after autopay discount: 1st. $60 (paid), 2nd.$60 (paid). 3rd $20 (free). 4th $20 (paid).

1. I am new customer to T-Mobile. do I get $200 costco GC if I open a new account and port my line over?
2. I have total 4 lines with AT&T. To be eligible for "free new line" offer, should I do all 4 lines at costco together or make a second trip after getting two lines opened as new customer?
3. If eventually I move all 4 lines to T-Mobile, how many lines will qualify as "free new line", 1 or 2?
4. I do plan to get $100 GC, so I should port my lines after coming back from Costco, correct?
5. Existing customers mentioned SOC code. what is it? do I need one for any of the above offers ($200 Costco GC, $100 T-Mobile GC, Free New Line)?

You might be able to get 4 lines for $120, with 2 lines free, and other options that you might be able to stack ($200+$100+free phones).
In terms of plans, Magenta is almost always better than Essentials. Includes taxes, better for promo eligibility. In high tax places Essentials might end up being more expensive than Magenta.
SOC is a code for the plan that you will get with T-Mobile. Magenta SOCs should qualify for most if not all current promos and will have better eligibility for various future T-Mobile deals as compared to Essentials, military, 55+, etc.

Edit: I have simple choice with 9 lines, SCFUTT6. I dont have a flexpay account. Not sure why it is identifying me as a flexpay customer.
(I know the free line promo does not.)
