I've read through the comments here but I'm not fully clear on how this works with Costco.

Do I have this right: I go to Costco, bring a phone and add a free line there, and then I get a $200 Costco card? Or what's the procedure?

Not necessarily. It depends if your plan has the fees and taxes included or not. I have Tmobile ONE, which includes all fees, therefore my existing free line is completely free...
