Porting in from various networks: don't use Google Voice, Boost or any MVNO that uses Tmobile as their network. Even if the rep let's you do it, your rebate will get rejected later.
It is not known if you can stack the costco $200 with the tmobile $100 but one thing to note is that the costco rebate is by Wireless Associates which runs the booths in Costco, while the $100 is directly by T-mobile.

I went to Costco and added a new line with port in from Verizon. All went well. But when I check my online account, I do not see "2020 Line on Us 3", but rather see "$4.00 New TMO Insider Voice 20% (Additional Lines)" added.

Interested in this too. When I was trying it, along with having to upgrade my account it wants to add 20 dollars a month for the line.
Now I see the new line in my Tmobile account shown as $0 and also submitted a rebates request.
BTW, I am not charged a penny for either SIM card or assistance fee.
