Just for some context for people who are wondering why their subscription isn't showing up after it says redeemed: i've used the free t-mobile mlb.tv account for the past few years, and clicking on "manage subscriptions"on my macbook always just results in a blank page--but the service works just fine. It was like this last year for me, too. Go back and see if you can watch some earlier spring training games or older content, if you can, your account is working fine.

When you first navigate to it, did the form show up? If it did and you tried signing up, go into your mlb account and check under manage subscriptions.

Never got a form, I just tried again and it just redirects to the mlb.com homepage and nothing else happens.
