The offer is not on the Costco/Wireless Advocates Website, but it was given to me at the kiosk and was told this is a national offer. The rebate paperwork I was given/submitted confirmed the above.
This is one of the more generous switch offers I've seen recently for BYOD, only higher offer I can recall was during the Black Friday Shopping Period when $350 was being offered to switch. The only higher offer in recent Happy deal hunting.

You definitely have to keep service for at least 14 weeks. But beyond that, you'll already have the cash card, so that's that, right?
