did this deal today, added 4 lines total. no need to port in numbers, can combine with add 2 get 3rd line offer.

Typically the restrictive language will drop your offer if you drop any lines from the account during the promotional period requirements.

The prepaid program explicitly does not throttle your speeds in the way that the postpaid plans do. Instead, the plan cuts you off and offers you the option of purchasing data passes:

Need membership, if you open 2 lines, costco cash card with cover your tmobile bill and costco membership fees. If you wish you can always cancel membership anytime for full refund.

I have 4 lines on VZW right now... can I just bring all four phones with me and get it done? and after I receive the Costco Card I can switch to Visible (after 14 weeks)?

I cancelled a line on my account just last week.
Any idea if I can still take advantage of this deal?
They usually mention that "if we cancelled a line within the last 90 days, we need to activate those lines first." I do not see that condition in the terms in the picture posted by OP.
