and 90 days period starts from the day of approval or day of receiving/activating the card ?

Well damn, that seems a little strict. I'm hoping my Chase Business Visa card will work as a second ID. I don't have a second picture ID. I assume they can expedite the shipping in branch. Or at least I hope so. Going tomorrow. This worked out perfect as I just received my $750 reward with Chase Business card and can even deposit the money into a bank account. Now in for another $800.

Yes they're able to expedite shipping in branch. The representative has to call in to make the request.

How do I find out exactly if I am over or under 5/48 ? I got my CSR in Jan 2017 but the 5/48 date is based on when the bonus was paid out. I don't have the card anymore.

Got 1, hope 80k points to come. I had my old sapphire card cancelled more than 6 months, plus I think last time I got the bonus was more than 4 years ago.

It does. If the value of your referrals is under $600, you don't get a 1099. That's usually the threshold required to generate it.
