I signed this up and it does not seem to conflict with the free 12 months offer with Chase.
I was able to get 3 months free plus $5 off 2 orders per month for 3 months. I used all pickup orders so it has been a great deal. The restaurant across the street does not markup the prices for Doordash. Some restaurants may have inflated prices on Doordash so YMMV.
I see no reason to keep it after 3 months as I use Doordash exclusively on pickup orders.
After 3 months it would be like paying $10 to get 2 $5 discounts every month, plus a 5% cashback on some orders. However, I don't like the hassle of having to place 2 orders every month just to get the money back.

Edit: In case anyone else has this problem I think I figured it out. I just had to start an order from a different restaurant and it loaded up the $5 off. I was trying the same restaurant and it was not loading. Not sure if I burned the first one and this is actually my second.
