Up to 830 is because if you buy 12mini it will have trade in credit or only $730 and not $830 and it you buy 12pro or pro max then you will only get $830 and rest will be on you. That's what upto $830 means so yes iPhone 8 will get you upto $830 depending on the device you choose

Where is Costco? They use to have similar deal as Tmobile but not lately. Wonder if we didn't kill the golden goose by canceling "new line" after getting the rebate.

For those of you trying to get a phone from SWAPPA like a iPhone 8 or S9 series, make sure you do order from an individual seller where the item/images clearly display the listing number.

In that case it's a unique listing offer and the IMEI will be displayed on the sales page right after your purchase. If you order good quality or spend $10 more for mint you should be golden. Once purchased on the sales page you will see the serial number (IMEI):

If you order from an enterprise seller with generic images and none displays the unique listing number the IMEI will NOT show up and you may have to wait until they are back in the office on Monday and the deal could be over.

Thank you very much SD community for all the helpful comments and tips. I was about to do this online on T-mobile website and read a comment about Costco doing it with $30 Costco cash card, went with that option.
Will be trading in two XRs for $159 each and ordered two 12s at Costco Wiresless kiosk. Got trade in shipping labels, shipping envelopes and paperwork.
No shipping charges when ordered from Costco. Activation fees waived off.

All done in about 30 min.

T-mobile account history correctly says-$27.96 credit for 24 months for each line.
(27.96x24=671.04
+159 XR trade in value
=$830

More information about trade-in screen/LCD condition. T-Mobile ask below question when you do trade-in online:

Does the device have an acceptable LCD and display?

If you click the more information icon, it says:
A device with screen damage may be eligible for a reduced trade-in credit but will not qualify for promotions.

Don't know what they define as "acceptable" and "damage". Are scratches fine? I guess it depends on the person who does the inspection. Also, even if the trade-in value is reduced, it is fine since the majority of the credit amount is in the monthly credit. But the "not qualify for promotions" is concerning. Hopefully all go well.

I think it does both essentially. They inspect in person but also again once the phone is received by them
