Wrong, you do not need to activate a brand new line. Nor do you need to pay for a brand new phone up front.

You don't need to activate a new line, and you can trade in a low-value phone like an iPhone 8 toward $830 off an iPhone 12. Many people have no plans to leave T-Mobile. That's why it's a front page deal.

The down payment applies to the cost of the phone and the remainder is financed over a 24 month period. I believe they deem it based on your credit score.

On the last part, it depend on which Costco you go.

Many in my area simply took the IMEI, and told me they do not inspect the trades as they do not want to be responsible.

I only showed them the IMEI on the phone box

I believe some credit cards actually offer phone insurance for free if you make payments with the card. However I never claimed such benefits so I don't know how they actually work.
