Same here. Here's the timeline for those who are curious:

6/23 made $50 transaction at T-Mobile. It is named "T-MOBILE POSTPAID WEB"
6/24 made another $50 transaction at T-Mobile. It is named T-MOBILE POSTPAID WEB"
7/3 got email that I redeemed the offer. Also says $100 Redeemed within the Chase app. (As others have commented I never got the initial push notification or email that I qualified for the offer once I made the second qualifying transaction on 6/24.)
7/6 the $100 is posted as a statement credit to my account.

I know this offer works on online purchase of accessories. Can anyone confirm will it work for store purchase of accessories as well? Some of the accessories that I want to purchase are out of stock online but available in store.

What do you mean? If you added the offer and used it. It will disappear after 7-14 days and in a couple of days move to the redeemed tab.
