The tool you would be looking for would be a leather punch. You would essentially put a block of wood on one side of the material, and then hammer a hole in it from the other. I don't know that it would look all that clean, but it would definitely be your best shot at ending up with a reasonable finish.

Well not every doctor gives the best advice. I think it's well meaning advice, but maybe another shoe has better support.

These in particular or just Crocs in general? If Crocs in general, I've found that they're not that unreasonably priced for something that lasts several seasons, and several varieties do have decent arch support. (I don't wear these particular ones, but I do like the Crocs that look more like real shoes and less like clogs.) That said, I have always found them at discount at TJ Maxx/Marshall's/whatever else they own. If you're in Dillard's country, Dillard's has a sale of some sort on them every once in a while if you're particular about looks/colours. There's always the amazon, of course. For the cost here or at one of the places I mentioned, why not just buy a pair and walk around your house to see if it works? I still kind of think you need a second opinion (or just to walk into a specialty shoe store and try on every shoe), but I'm also not a doctor.

While that's true, don't you think it's still worth purchasing the Crocs & seeing how comfortable they are for you first? Best case scenario, they're awesome for you & worse case, you're out $30?

Your doctor is really just suggesting cork or eva cushioning that will conform to your feet, check out online podiatry articles about it. You can search for Boston clogs or Arizona sandals to get the styles that are similar to Birkenstocks but in cheaper brands.

My altras have a thick eva midsole and wide toe box and are quite comfortable, but totally not the style that your doctor suggested. But just to say all that foam is very soft and pliable.

I got these a couple weeks ago as my first pair and man my feet get moist. The main reason I got them is for going outside to chase cats out of my yard. I have a ton of those little arrow shaped stickers from wild grass so I didn't want holes. They're great for quick dashes outside but definitely don't want to wear an extended period.

A leather punch isn't going to work well here.

Best bet is probably a series of increasing size drills, go into the material slowly but with the drill spinning fast. A router bit might work better.

I have really good luck with my new balance 860's. Prior to these I used to use Powerstep Pro insoles.

If you are flat footed and need support, don't get a sandal or clog.
