The offer is valid from 6-7-2021 to 9-10-2021 and only to the first 50,000 people. They don't give you the discount right at the cash register... they give you a $20 credit at some point in the future *if* you are the first 50,000. So it's impossible to know if you are going to get the $20 or not. Back when they had the $10 back on a $20 transaction, I had to contact the Better Business Bureau to get the $10 from PayPal. I advise you to steer clear of this one; PayPal doesn't have the best reputation at fulfilling promotions like this.
I bought two $25 Amazon gift cards in two separate transactions. Paid for one with PayPal ($20 off) and one with Venmo ($20 off), then PayPal sent me $10 cash back which covered the remaining $5 for each transaction that I paid.
I saved both to my PayPal account and they both automatically applied. $20 off applied instantly to transaction, and $10 offer was applied as cash back a couple minutes later.
The $20/$10 CVS offer is in my paypal acct but it doesn't allow an option to "save" it. When you click on it then it takes you to a page that explains how the offer works. Is it automatic or do you really need to save it?

Was the $10 off $20 offer showing in your acct as a saved offer? I activated it when it first showed up, but it disappeared from the saved offers. The banner still shows on the Paypal offers landing page though with no sign up option.
I scanned the QR code at home into the Venmo app. At the store, I selected a $25 Home Depot gift card. I put on the screen the "use at checkout" QR code (or however it was worded) from the Venmo app in front of the live cashier. She scanned it, and the transaction went through.
When I got home, I checked the Venmo app, and it merely charged me $5, invisibly and automatically deducting the $20 CVS credit.
Great deal!

For those who, like me, previously received the $10 back after spending $20 on Venmo, I just bought a gift card using PayPal and got both the $20 off and the $10 back. Good luck!

No, it didn't have an option to save the offer for me. It was automatically applied.

If you're shopping online, just go to the CVS website and checkout with PayPal.
If you're shopping in the store, choose Scan/Pay from your PayPal app to pay with a QR code.
