Click here and login to your existing account or to sign up for a free account
Click here after logging in to your account and click Add Delivery Address
Fill out your delivery information and click Next
Click Manage My Memberships
Click Upgrade to Premium Membership
Under Do you have a promotion code? click Yes and apply promotion code CVSMYCHOICE10
Your 12-Month My Choice Premium Membership will be $10
