UPDATE: I see lots of confusion here and wanted to update the OP.
First off, I am a Costco guy myself and wouldn't pay for a Sam's Club membership.
If I get a $45 gift card for a $45 membership - it's FREE. I don't understand the debate.
On top of it, Amex Offer has been pretty reliable for me. And if it says $25 statement credit is on the table, I don't have reason not to believe it.
Update 2: data points suggest this deal does stack. Many users reported getting both Sam's gift card and Amex credit. It does take a loooong time, 5 weeks for the sam's gift card and 30 days for amex credit for me personally.
Stack this with Sam's club new membership gift card (between $20 and $45 gift card).

There are few products and services that a membership is worth it for , but most of their products , should not require membership. Same with Costco
If you sign up with the $45 back or Uber Voucher promo the AMEX offer is a bonus . I would not cancel. I would complain to AMEX
