@badboy2man look at your packing list in your online Kohls account, see if that shows 11/21, if it does, print those out and use those to submit the rebate. They are acceptable to use for the rebate T&C.

You need ONE rebate form per manufacturer. If you only bought Toastmaster or Hamilton Beach items, then you only need THAT rebate form for ALL orders containing Tmaster or Ham Beach appliances.
The original UPC is taken from the box of the product, usually on the very bottom. Use a box cutter or sharp scissors to cut far enough in around the UPC code to lift it off. You don't necessarily have to make a gaping hole in the box to get the bar code off the box. Especially if you're gifting it or reselling it.
If you bought multiples of any given appliance (say 5 coffee makers) staple the 5 UPC's from the coffee maker together so it's easier for the rebate house to verify your purchases if you got 5 coffee pots, 3 toasters and 3 blenders. (staple each set uniquely by item, don't mix them all in one big pile). then staple each set to the side of the rebate form and attach the packing lists or store rebate receipt behind the rebate form.
Copy everything and mail it with adequate postage if heavy envelope. Each manufacturer rebate form MUST be mailed in seperately. But if you have 4 different orders each with Hamilton Beach on them, you can submit ONE form with 4 orders/upc's on them, as long as you don't exceed the limit 5 rebates of any given item.
No. It does NOT. I did the same deal last year. Paid with KMC ($600+) only!. I received both kohl's cash and rebate without any issues.
Update: I am sorry I misread KC as KMC. Anyways, KC should not negate the rebate. I had a chat with a supervisor from kohls rebate team last year. That time I specifically asked if using KMC would negate the rebate and she said any mode of payment is acceptable.

The rebate amount said $220 instead of $370.
I contacted them via chat and the lady fixed it right then and there without requiring any proof or discussion. You just gotta be patient and polite. I did have to correct her once (she initially counted $360 instead of $370)
Note that I properly taped the UPCs to regular printer paper, separated by items, so it'd be easier for them to calculate.
