E911 availability depends on the VoIP provider. The feature can be added to most plans very cheaply, if needed. I suspect most of these devices are used to keep an old landline phone number alive with Google Voice and not used as the only phone device at a house.

You can use your Google voice number. Check if you can port your spectrum to google voice somehow.

IIRC Google Voice only allows porting in from mobile numbers. It's been a few years since I was involved in a landline port, but the cheapest/easiest path for a non-mobile phone number was to get a TMobile SIM, port your number to it, then port from TMobile to Google Voice. Took a few days, but was straightforward.

Alternatively, you can port a landline directly to a VOIP provider and then use this device for very low cost calls. It's not free, but then again, you are not giving Big G more of your data with this path. The site nerdvittles.com is a good source for info on VOIP providers.

When I bought my first OBi device back around 2011, I would have been really surprised to know that in 2021 they aren't more commonplace. It baffles me why there continue to be so many on SD who are still paying big bucks monthly for landline.
