Did this myself about a year ago:
For Amazon packages, there likely won't be a UPC label on the box, instead it is ASIN. I basically carved the whole shipping label used by Cooler Master to send to Amazon containing the ASIN out of the box and sent it in. The mail in rebate card is from Wirecard which has issues integrating with old online database (it's now North Lane cards). Online activation of the mail in rebate card failed repeatedly, I think I ended up using it to reload my Amazon wallet balance.
