I switched my phone number from Cricket at a Corporate store. I paid $99.99 (iPhone 11) + $60 (service) + $8.25 (tax) = $168.24 total

Rep told me that I can still get $50 GC, if I maintain the service for 3 months and also I can change plan from next month onwards to lower plan from $60 to $40.
Good luck on that rebate. When they offered $150 for 12 mini I had to fight them for it for hours on the line and ended up staying for a 4 months due to them saying it needs time to be processed which took over weeks to happen.

Can this phone be unlocked for Verizon MVNO?
You can use it with Verizon or MVNO once unlocked.

The rebate isn't worth it unless staying long term with Metro.

How do you do unlock? Instead of waiting for 180 days.

I was told white SIM is good for GSM unlock only. Can't Be used with Verizon???

No matter you are the T-Mobile customer or not, only matter is have you done any Metro deal within 180 days and the port in number can't be in T-Mobile network past 180 days.
YMMV, but I had switched to Verizon 2 months ago from T-Mobile and it was not a problem for me to port in and get this deal.

Your number was on T-Mobile 2 months ago, and you managed to port that number over to Metro for this deal?

I wonder if you got lucky, or if they've changed the rules. I tried some time last year with a number that was on T-Mobile ~3 months prior, and got rejected.

Correct, I had switched over to Verizon from T-Mobile sometime at the end of December. I guess it depends how you interpret their T&C: "Excludes numbers currently active on T-Mobile or active on Metro in past 180 days", but perhaps if you are not currently active on T-Mobile you should be good to go?

Is there a link to fill out rebate online? The store did not provide any form or detail on how to get the rebate.

1. Use $60 plan for three months,
2.keep SIM card in same device you as you purchase
after that you will receive text message regarding how to redeem the rebate, if you don't meet the requirements, you won't eligible for the rebate.

Is this the right one to buy? Or do you have a link to another place where we can get white sim?

It's soft unlock if you use the white sim. Every time you reboot, gotta do the whole process again. Works with every carrier though.

Permanent unlock is only waiting 180 days.
