- A one-time $20 activation fee applies
- Remain active / pay for 3 consecutive months on the $60/mo. plan

Unless you didn't give any of your ID the first time you did it. I know some users that were able to do it more than once going to multiple stores since they were never ID'd the time they ported.

Sorry. I don't have this so I can't confirm if it would work with Verizon/Visible. I know these phones work with Verizon/Visible once it's perma-unlocked since I tested it with the OnePlus metro deals.
