Will this have any repercussions if my insurance knows I've been exposed? Or if the government knows I've been exposed? How is this info used?

I would like to take the survey to see if I am eligible for $0 upfront cost to bill my insurance or access federal funding for uninsured.

They are billing your insurance. This is not free

Was that really necessary? Giant text? "Provide insurance information or an acceptable ID to receive an at-home COVID-19 test collection kit for $0."

I don't know what "acceptable ID" means. Insurance companies and health plans are required to cover 8 free over-the-counter at-home tests per covered individual per month.

It wasn't actually meant as a criticism…the Giant text was to make sure people understand that these are not like the free tests being provided via USPS…

New laws make sure that Insurance covers over the counter tests, so this does make sure you don't have to upfront the cost. So if you truly were exposed or whatever, this is easier than buying and then claiming. Acceptable id means a license or something plus you claiming to be uninsured (don't know what the downside of that is - if you are actually insured).

Yes and No…I mean they will know you have been possibly exposed - they are not asking you to share the test results so they don't know for sure…same as if you went for an annual checkup and they found out your cholesterol is a little high or something…so no major thing that way…
