How could anyone damage their eyes from the resolution?

Are the pixels too jagged and you cut your eyes on them or something?

Would this be good for my parents that is in their 70s, they can't see well on the 20" @ 1280x1024 and using the larger fonts mess with web layout, I feel a larger screen with lower res (non 4k) would be better ?

I would rather buy a higher resolution screen and increase scaling within the operating system. That way everything would be large and clear. There are some 32 inch 1440p monitors for a bit over 200 dollars at the moment.

YES it will help them. I use 27" 1080p for my parents. Even so large fonts needs to be changed to.
