I am still awaiting approval for the tmobile cash card deal.
Per Costco employee I am in touch with: The Costco corporate office took control of the list of all shop cards owed to members company wide on 12/8. They said that they are in the process of sending them out in the mail as soon as possible.
We are aware that some Costco members have not received the promotional Shop Card associated with a purchase and activation of a mobile phone through Costco that are past due. We are working on a resolution to the issue.
Shop Cards that are past due will be issued to all eligible members as soon as possible.
If you have already received the shop card, please disregard this email.
We apologize for any inconvenience this may have caused. We value you as a customer and appreciate your business.

Quick question to the ones who received email ..seems like it was only sent out to those that are past due i.e rebate submitted 14 weeks ago.. correct?
Mine was submitted a few weeks back and I was prepared to wait until March .. would be great if they choose to pay out sooner and be done with it😁
