look at 3 attachments from when i did this deal a few months back.
in bay area , ca
when i had only 1 line.===>total was 16.44
when i had 5 lines===> total was 21.75
in middle when i added 2 lines on 8/24 and 2 lines on 8/26.
the bill as crazyhigh.
and this was after getting a 60 courtesy credit !!

So it may be you ported some tmobile lines
For me i do see:

When I tried activating, it doesn't allow me to change the Port in number and the Port in provider.
It only allows me to fill the Account number and PIN. Thanks.
