Do I need to pay more to T-Mobile to activate? Never had a smartwatch. I currently have T-Mobile family plan for 2 lines for $100 all inclusive price, have Samsung 21 phone.

Yes. You'd need an additional data package. They (cell providers) have dedicated data packages for smart watches/tablets.

Without paying anything extra, your smartwatch will pair with your phone via bluetooth and access the internet via wifi connections to act as an extension of your smartphone.

For about $10/mo, t-mobile will add a cellular data plan directly for an LTE watch and pair the watch with your phone's number. So you could leave your phone at home (to go for a run or bike ride) or your phone's battery could die, and calls and texts and some notifications would go directly to your LTE watch without any bluetooth or wifi connection. This last one is the reason some people are willing to pay a bit more to get a smart watch with LTE/cellular.

I had one which I returned to Samsung. I was getting a rash on my wrist due to sensor getting heated. Search for this issue on Google before buying

One minor correction: Unless something has changed with T-Mobile's watch plans in the past year, the phone has to be on and connected to T-Mobile's network in order for calling and SMS to forward to the watch.
