Every brand have a different fit, going by size does not work well here especially going by US sizing.
I would point out at the top is says:

Been using Sidi for decades and have tried many other brands in that time.
Have never found any that fit as good.
They are pretty consistent with sizing so if you are happy with them I say stick to them.

You could check if any shops near you carry them and go try them on.
If you find it's a good fit, this is a pretty good sale, worth a shot.
I was worried about the fit too due to the "no returns" policy. Luckily one shop near me had the MTB version of the CX 218 in WIDE available to try. Hopefully this helps you out: I'm usually comfortable in a size 12 street shoe size. On the online size chart, I was a 45 and standard width based on 283mm foot length and 100mm measurement. When I tried on the 45 size in store, it was just a smidge small, width was good, 46 was too long, 45.5 was perfect. I was surprised that 45.5 in WIDE was the best fit.
TD:LR: In my experience, go up half a size from the chart and add 10mm to width.

I was getting a hard time from my team mates until we are at an event with the Olympian Nelson Vails, who just so happened to be using the pedals, clips, straps, and shoes that I used - as we discovered when he chose my shoulder to lean on before the start of his race because he was strapped in and didn't want to remove his foot from the pedal.
